Post Permanency Services Through The Statewide Adoption Network (SWAN)
 

Family Services of NW PA is a SWAN approved agency that is authorized to provide Post Permanency Services to any family that provided permanency to a child. The type of permanency includes:

  • Adoption (international, private or child welfare/SWAN)
  • Guardianship
  • Permanent Legal Custodianship (PLC)
  • Subsidized Permanent Legal Custodianship (SPLC)

Post Permanency Services provided by Family Services of NW PA include: Case Assessment, Case Advocacy and Support Group Services.

If the child you have in your custody due to one of the permanency options listed above is experiencing mental, emotional or behavioral problems, you are eligible for this service.

What Is The Process?

  1. Call the SWAN Helpline at 1-800-585-SWAN (7926). The Helpline staff will explain the services, assist the family in choosing an agency to complete an assessment and submit the referral to that agency
  2. If selected, a Family Services of NW PA Resource Family Caseworker will contact you to begin the Case Assessment Process. They will meet with the entire family to identify and build on family strengths. The worker will then recommend the best possible services to fit the family’s needs
  3. The array of services that Family Services might recommend includes Case Advocacy and Support Group
    1. Case Advocacy is designed to provide support for families through a case advocate who communicates with the family by providing ongoing support. The advocate also assists the family by attending meetings or helping to access services in the community
    2. Support Group service is designed to ensure that families are supported by building relationships with other families, forming a community of support and mentoring and providing information and educational materials
